On the day you get your braces you will be given an orange fluoride card and some starter Levine Loot. The orange card is a calendar for your fluoride use at home and it is a report card for your oral hygiene each time you come to the office.
First: Bring in your completed orange calendar and you'll get LEVINE LOOT
Second: Earn an A for oral hygiene by really working to take care of your mouth in between visits and you'll get LEVINE LOOT
Third: Each visit that you come in with nothing, zero, nada, nil broken you'll get LEVINE LOOT
You can earn up to 3 dollars of LOOT each time you come and then redeem it for some great things like Blockbuster Gift certificates, Rita's Italian Ice, Dairy Queen Cones, CD Warehouse certificates or Movie Passes.
